This quick reference contains information about requirements to qualify for the Medicare and Medicaid EHR incentive program. Complete the checklist to ensure meaningful use of the electronic health record (EHR) technology. You must exceed the Pass% for the specific report to qualify.

Percentage of permissible prescriptions written by the Eligible Provider (EP) and transmitted electronically. From FNC > Rx, order medication for a patient electronically or refill a medication for a patient from
the Message Center.

Medication List
Percentage of patients seen by a provider who have at least one active entry, or an active indication of no known medications recorded. > Schedule an appointment
> From FNC > Rx or FNC > Hx > Medication History, record medication information for a patient or indicate that the patient is not taking medication.

Allergy List
Percentage of patients seen by a provider who have at least one active
medication allergy, or an indication of no active medication allergy,
recorded.

Demographics
Percentage of patients seen by a provider for whom you recorded demographics for race, language, ethnicity, date of birth and gender, or an indication that you were unable to obtain this information. The patient might refuse to provide the data or requesting the data might be prohibited by your state. > Schedule an appointment

Desktop > Find Patient > Select Patient Name, record demographics information
for a patient or indicate that you were unable to obtain the information.

Vitals
Percentage of patients seen by a provider who are 2 years of age or older and have height, weight, and blood pressure recorded. > Schedule an appointment

Smoking Status
Percentage of patients seen by a provider who are 13 years of age or older and have a smoking status recorded. > Schedule an appointment

Lab Results
Percentage of clinical lab test results ordered by the provider during the reporting period and recorded in a positive/negative or numerical format. > From SO tab or SP tab or Lab Order Message, create a lab order.
> From the Results Status Screen, enter lab results for the tests.

Patient Reminders
Percentage of patients 65 years old or older, or 5 years old or younger, who were sent a reminder during the reporting period. > From List Editor > Demographics
> Communication Preference, set up a communication preference for the patient.
> Send a reminder using the preferred mode of communication.
> From Desktop > Tools > Patient Reminders, enter the date the reminder was sent in the Reminder Sent field.

Electronic Copy of Health Information on Request
Percentage of patients who request an electronic copy of health information and receive it within 3 business days. > Receive patient request for electronic copy of health information.
> From the Release of Information window, generate a Clinical summary. Click Print, Save, or Export. Export sends the summary to the Portal.

Electronic Access to Health Information
Percentage of patients seen by the provider who have electronic access to health information within 4 business days of updates being made to the electronic health record (EHR). > Schedule an appointment.
> Create and save the patient visit note.
> Create a Web account for the patient, which uses the e-mail address of the patient as the username. The password is automatically generated.
> Patient health information is automatically accessible from the Portal.

Patient Visit Summary
Percentage of office visits resulting in the patient receiving a clinical summary within 3 business days. > Schedule an appointment.
> From the Release of Information window, generate a Clinical summary for the patient. Click Print, Save, or Export. Export sends the summary to the Portal.

Medication Reconciliation
Percentage of transitions in care to the provider for which medication reconciliation was performed. > Schedule an appointment.
> If the patient is being transitioned
to the care of the provider, note the transition in the Hx tab or from the Visit Checkout window.
> From Hx tab, check the Medication Reconciliation Performed check box to indicate that the Medication Reconciliation process was completed.

Patient Education
Percentage of office visits resulting in the patient receiving education materials specific to the patient's needs. > Schedule an appointment.
> From the Education Form Slider, view the Patient Education Form. Click Print to print the form.
